There are time limits for making a Service complaint and requesting a review of decision or investigation by the Ombudsman. Simply contacting our office will not “stop the clock” on these time limits. Please read all of the information provided under each section before submitting your application.
It is important that we can contact you once you have submitted an application. If we can’t contact you we may be unable to process your application or there may be delays in making a referral or starting an investigation.
Please note that if you are sending your application by email that the system cannot accept emails larger than 20MB. Emails that exceed the size limit will be blocked by our email system.
Even though you can access our forms on smartphones and tablets, you will need to have a compatible smart phone app to amend and submit the form. E.g. Adobe Acrobat Reader, which is free.
If you don’t have one of these apps you will need to use a desktop or laptop to complete your form.
